Lancashire and South Cumbria Integrated Care Partnership - Monday, 16th December, 2024 10.00 am

This meeting will discuss the Lancashire and South Cumbria Integrated Care Strategy, data strategy and public engagement. The Integrated Care Partnership will hear presentations from local and regional health officials on its plans for patients at the end of their lives and will hear about public feedback gathered about the local NHS.
Transforming Community Care
The meeting will receive a report summarising discussions that took place at the previous meeting on 7 October about the Transforming Community Care Programme. In the report, the Integrated Care Partnership lays out its ten key goals for transforming community care:
- Maximising use of community assets such as Whyndyke Farm in Fleetwood.
- Ensuring that it is easy for people to access services.
- Providing person-centred care.

Moving away from crisis-driven, bed-based care
. -
Sharing records across health and social services
. - Improving the use of digital technology.
- Creating
a community-oriented
workforce. - Improving end-of-life care.
- Empowering communities
to take control of their health
. - Measuring the success of the scheme by looking at specific outcomes.
The report also identifies eight priority actions that will help to achieve these goals:
- Making good housing a priority.
- Creating shared records.
- Implementing Care Coordinators.
- Addressing inequality.
- Improving partnerships and collaboration.
- Strengthening the role of the Voluntary, Community, Faith based and Social Enterprise (VCFSE) sector in transforming community care.
- Data sharing.
- Accountability.
The report also discusses the role of the Integrated Care Partnership in transforming community care.
Update on the Dying Well Domain of the Integrated Care Strategy
Sue McGraw, from the Lancashire and South Cumbria Hospices Together collaborative, will present an update on the work being done to deliver the 'Dying Well' Domain of the Integrated Care Strategy. The presentation will focus on non-clinical areas of work being done by voluntary, faith and community-based organisations. In particular, it will focus on work that supports the nationally mandated 'Getting to Outstanding' programme.
The Integrated Care Partnership's ambition is
to get the people of Lancashire & South Cumbria comfortable with talking about and planning for dying and then well supported in bereavement.
The Partnership has three strategic goals for end-of-life care:

Get the people of Lancashire & South Cumbria comfortable with talking about death and dying.
- Make sure
end of life care will be personalised \[...\] regardless of where they live or their condition
. - Provide
outstanding bereavement support for people, their families and carers in our communities.
The Partnership has identified seven actions to help achieve these goals.

Compassionate conversations
, including training for friends, family, care staff and health staff provided by St John's Hospice in Lancaster. - Increasing the number of people who have end-of-life conversations and decide what they want to happen to them in their final days.
- Supporting public health partners to
promote end of life care conversations/plans, and bereavement support
. - Increase the number of volunteers trained to support people at the end of their lives.
- Create a
map of bereavement and \[end-of-life\] services
. - Develop a
Bereavement Improvement Plan
. - Investigate how
communities/volunteers can support bereavement services with compassionate conversations
.
The partnership will measure the success of its work by looking at:
- The number of people who have end-of-life conversations with their GP.
- Whether each Place has access to bereavement support.
Lancashire and South Cumbria Integrated Care Board (LSC ICB) Digital and Data Strategy
Linda Vernon, the Head of Digital Empowerment at the Lancashire and South Cumbria Integrated Care Board, will present an update on its Digital and Data Strategy. The report outlines the strategy's mission, which is:
[t]o improve the health and well-being of the 1.8 million population of Lancashire and South Cumbria through the supported adoption and use of consistent, increasingly powerful digital and data systems, tools and services accessible in all our communities.
The Partnership's vision for digital and data is that
[d]igital and data underpin the transformation of care and care pathways to improve the health and care outcomes throughout Lancashire and South Cumbria.
It has three goals for how it will use digital and data:
- Empower the region's population by providing
innovative and effective service provision
. - Make sure that the region's health and care workers are
digitally skilled and data informed to confidently design and deliver agile and effective care
. - Fully integrate its partners into
a coherent set of systems
.
The partnership has identified four digital and data priorities:
- Create a single digital infrastructure.
- Implement a
single set of core strategic system platforms
. - Create a single data architecture.
- Develop a
single digital & data service delivery & support model
.
Update on Public Engagement and Involvement & NHS Change
Neil Greaves, the Director of Communications and Engagement at the Lancashire and South Cumbria Integrated Care Board, will present an update on the 'Change NHS' campaign.
The Change NHS campaign was launched by the Department of Health and Social Care and NHS England in November 2024 to help develop a new 10-year plan for the NHS. The Integrated Care Partnership submitted its response to the consultation, after consulting leaders from partner organisations, and highlighted three key themes:
- The need for a multi-year plan to allow the NHS to transform itself.
- The importance of national decision-making to support local systems.
- Recognition that the NHS cannot solve public health issues in isolation and that it will need to work in partnership with local organisations.
The Integrated Care Board also undertook a programme of public engagement between September and December 2024. The programme heard from over 1,800 people and involved a series of public meetings, targeted engagement with community groups and networks and surveys. The Partnership used the programme to discuss its vision for the local NHS, which is that:
[b]y 2035, Lancashire and South Cumbria will have a high-quality, community-centred health and care system, focusing on prevention, wellbeing, and early intervention.
The partnership also used the engagement programme to discuss its three priorities for the local NHS, which are:
- Creating
sustainable, integrated clinical services

Shifting care to the community
, particularly mental health and autism care. -
\[I]mproving quality \[and\] resource use
.
The Integrated Care Board has produced a report summarising the feedback it received. The report, which can be read on the Integrated Care Board's website, highlighted a number of concerns raised by the public, including:
- Long waiting times.
- Overcrowding and capacity issues.
- Insufficient resources and funding.
- Poor quality of care.
- Challenges in accessing specialist services.
- Neglect of vulnerable populations.
- Bureaucratic inefficiencies.
Mr Greaves will also outline the Integrated Care Board's plans to engage the public about its New Hospitals Programme between January and March 2025. The engagement programme will involve public meetings, roadshows, targeted engagement with marginalised groups and market research.
Finally, Mr Greaves will discuss three key considerations for future partnership working on public engagement.
- The need for engagement to take place in local neighbourhoods.
- The need for the partnership to be able to demonstrate that it has taken action as a result of public feedback.
- The need for a coordinated approach to public engagement.
