The Health and Adult Social Care Policy and Accountability Committee were scheduled to discuss an update on adult social care, co-production in adult social care, and the Hammersmith and Fulham Food Plan 2025-2030. The committee was also expected to appoint co-opted members and note the dates of future meetings.
Hammersmith and Fulham Food Plan 2025-2030
The committee was scheduled to discuss the Hammersmith and Fulham Food Plan 2025-2030, a five-year plan created by Hammersmith and Fulham Council and the H&F Food for All Partnership. The plan builds on the H&F Food Matters: Action Plan 2019-2024 and aims to:
- Reduce long-term causes of food insecurity
- Create a sustainable local food culture
- Lessen reliance on crisis support
- Promote healthier lifestyles
- Enhance access to nutritious food for the community
The plan has four key priorities: food security, healthy eating, connecting communities and sustainable food systems.
The report pack notes that in March 2025, H&F received a leadership and most improved Council Award at the Good Food Local report launch at London City Hall. The awards are part of the annual Sustain London report, backed by the Greater London Authority and Trust for London, which assesses each borough's food partnerships and efforts to tackle food insecurity.
The report pack also includes an Equality Impact Analysis, which concluded that the plan would have positive consequences and no direct negative implications for groups who share protected characteristics as listed under the 2010 Equality Act.
The report pack also includes a Climate Implications Toolkit, which concluded that the plan would have no direct negative implications for the climate.
Adult Social Care Update
The committee was scheduled to receive an update on the Adult Social Care transformation programme. According to the report pack, in May 2024, Children's Services, Adult Social Care and Public Health were brought together through the creation of the People's Directorate.
The report pack states that alongside a CQC Assessment, a professional assessment of the service has been undertaken, as well as extensive engagement and listening with key partners and stakeholders across health, the voluntary and community sector, providers and residents

The report pack notes that the assessments and engagement recognise that Adult Social Care requires improvement.
The report pack identifies the following priority workstreams:
- Prevention and early intervention
- Locality working
- Digital and data
- Workforce
- Health partnerships
- Medium Term Financial Strategy
The report pack includes a presentation that sets out how the council wants to help people to be independent and in control of their life, and to maximise their full potential.
Adult Social Care Co-Production
The committee was scheduled to receive an overview of co-production work in Adult Social Care. The report pack states that in 2017 the council set out its commitment of doing things with residents not to them, through establishing dynamic and innovative ways of working with residents, the community sector, and providers, to design and deliver services.
The report pack notes that feedback has indicated the strengths of Adult Social Care in listening and engaging with residents, however, it also recognises that implementation has been slow, and the priority is now to ensure that coproduction becomes an active process that delivers tangible outcomes.
The report pack lists examples of positive work across the department, including:
- Transitions
- Direct Payments Steering Group
- Mental Health Supported Housing Club
- Rethink
- Independent Living Delivery Group
The report pack states that officers are progressing the establishment of a new Partnership Board, which is being developed in collaboration with community and statutory stakeholders. The board will take a whole system approach, from prevention to longer term support, supporting enabling residents to live and age well in the borough.
Other items
The committee was asked to appoint Victoria Brignell from Action On Disability, Lucia Boddington, and Jim Grealy from H&F Save Our NHS as non-voting co-opted members for the 2025/26 Municipal Year.
The committee was also scheduled to note the following dates of future meetings: 17 November 2025, 27 January 2026, and 22 April 2026.
