Summary

The Health Select Committee was scheduled to discuss a range of topics, including updates on community healthcare contracts, reablement services, performance indicators, and the progress of the new integrated care centre in Trowbridge. The committee was also expected to review and approve its forward work programme.

Integrated Community Based Care Contract Update

The committee was scheduled to receive an update on the Integrated Community Based Care (ICBC) contract with HCRG Care Group Ltd, which began on 1 April 2025. The report pack included an overview of the contract's mobilisation, its management and oversight, and the safe transfer of services to the new provider.

The ICBC Programme Board, with representatives from Bath and North East Somerset, Swindon and Wiltshire local authorities, oversaw the contract mobilisation. The ICBC Collaborative Oversight Forum, a commissioning forum including Wiltshire Council representatives, now manages contract oversight.

According to the report pack, approximately 1,950 staff members transferred to HCRG Care Group from various providers across Bath and North East Somerset, Swindon and Wiltshire. This included around 1,300 colleagues from Wiltshire-based services, such as Wiltshire Health and Care, Medvivo, and Avon and Wiltshire Mental Health Partnership NHS Trust.

The report pack mentioned that HCRG Care Group has a four-phase transformation programme (Discover, Define, Design, Deliver) and that there are nine projects within the initial Transformation Programme, covering areas such as:

  • All age Single Point of Access with Care Coordination and Digital Front Door
  • Digital Innovation
  • Integrated Neighbourhood Teams
  • Partnership development and community engagement strategy
  • Community Estates Strategy
  • Clinical pathway and service transformation / harmonisation
  • Service identity and brand development
  • Workforce transformation (cross cutting)
  • Outcomes framework development and benefits realisation (cross cutting)

The committee was asked to note the post-mobilisation governance arrangements, the safe landing arrangements, the operational update of HCRG's first two months, and the development of activities to progress the intended transformation benefits.

Home First and Reablement Update

The Health Select Committee was scheduled to receive an update on the Home First/Reablement discharge pathway service in Wiltshire, focusing on demand and activity between April and June 2025. The report pack outlined service delivery, patient outcomes, and operational capacity since Wiltshire Council retained the entirety of the Home First Service.

The Home First approach aims to support individuals to return home from hospital as soon as they are able. The report pack stated that this approach offers benefits such as improved physical and mental health, reduced risk of hospital-acquired infections, and faster recovery.

The Wiltshire Home First Reablement Service (WHRS) provides support to assess and assist individuals discharged from acute hospitals. The service identifies needs for longer-term support and provides information and advice for people who are assessed as needing to pay for their own care.

The report pack stated that the Reablement pathway has undergone significant transformation, integrating staff from the previous provider and expanding the existing model.

The committee was asked to note the progress since the implementation of the new service, the increase in acute hospital discharges, and the 91-day outcomes. They were also asked to endorse the proposed future priorities for the service, including continued investment in workforce development and improvements in discharge timeliness.

Update on Trowbridge Integrated Care Centre

The committee was scheduled to receive an update on the progress of the Trowbridge Integrated Care Centre, which is due to open in 2026. The new centre will replace the nearby Trowbridge Community Hospital and deliver the ICB's long-term strategic vision of putting more hospital-level services in the heart of local communities.

The report pack stated that the new centre will replicate the success of its sister site in Devizes, and act as a hub for multiple health and care services, with teams from different organisations working together under one roof.

The report pack stated that the £16 million centre represents a significant investment and will bring benefits to people living in Trowbridge and its surrounding areas, such as:

  • Relocating all health and care services currently based in the now outdated Trowbridge Community Hospital to an ultra-modern health and care environment
  • More space dedicated to same-day urgent care
  • An improved parking experience
  • Creating a hub for multiple health and care services

The report pack stated that the centre will be one of the first buildings delivered by NHS Property Services, which manages the national NHS estate portfolio, to be 100 per cent net zero.

Adult Social Care Performance Indicators Quarter 4 2024-25

The committee was scheduled to receive an overview of the Adult Social Care Key Performance Indicators for quarter 4 (January to March 2025).

The report pack included data on:

  • The percentage of people in their own homes 91 days after entering the reablement service
  • The percentage of Section 42 Outcomes Met1
  • The number of adult social care care home providers currently rated inadequate in CQC inspections2
  • Long-term support needs of younger adults aged 18-64 met by admission to residential and nursing care homes, per 100,000 population
  • The number on the Housing Register
  • Total households in temporary accommodation

Forward Work Programme

The committee was expected to review and approve its forward work programme. According to the report pack, the committee was scheduled to discuss the Integrated Care Strategy (ICS), smoke free generation, NHS dentistry, oral health promotion, and the Older Person's Accommodation Strategy at future meetings.

  1. Section 42 of the Care Act 2014 places a duty on local authorities to investigate when abuse or neglect of an adult at risk is suspected.

  2. The Care Quality Commission (CQC) is the independent regulator of health and social care in England.
