Purpose
Bristol City Council aims to ensure a cohesive
and seamless approach to hospital discharge, working in partnership
with Sirona Care
Content
To recommission the Reablement Bridging
Service for 9 months, funded by the Discharge Grant 24/25 which
sits within the Better Care Fund (BCF) policy and planning
framework. This service will support National condition 3 of the
BCF:
•Build additional social care and community-based reablement
capacity.
•Maximise the number of hospital beds freed up.
•Deliver sustainable improvements for patients.
The service aims to support timely discharge from hospital and post
discharge reablement and intermediate care services, maintaining
and enhancing peoples’ independence where possible, and
reduce requirements for long-term care packages. The current
contract expires on 2nd October 2024.
