To award contracts for the delivery of
NHS Health Checks from 1 April
2024 to 31 March 2025 with an option to extend for a further
year to 31 March 2026to the following GP Practices organised into
nine Primary Care Networks (PCNs):

The total annual value of all
contracts is £245,000 and the total maximum value of all
contracts including the one-year extension is
£490,000.
