The interim Director of Adult and Health has
noted the:-

Agreement to implent a joint policy with partners across the
integrated health and care system in Leeds that supports discharge
from hospital and integrated care settings, giving clarity to staff
and people who use services about the purpose of their stay in such
settings and the next steps that will be taken.
