1.1 That the current Pilot to be extended from
the 7 of October 2023 to March 2024.
1.2 That to call-off and award a block
contract for the delivery of an End-of-Life support service from
MKCC’s Home Care DPS for maximum period of two years, from
April 2024, subject to continued funding, be approved.
1.3 That the Council enter into a service
level agreement with CNWL, which shall cover: (a) the current pilot
the Council commissioned for the delivery of an End[1]of-Life support service to a
cohort of patients identified by CNWL; and (b) the new call-off
contract to commence in April 2024.
