Purpose

The NHS Sussex Integrated Care System (ICS) is
a partnership that brings together NHS organisations and local
authorities, including West Sussex County Council (WSCC), to plan
services across geographical areas. The Council, working together
with the Integrated Care Board (ICB) and as part of the ICS has
developed a plan for the commissioning of hospital discharge to
ensure there is a sufficient level of service provision available
to facilitate time-efficient discharges from hospital for
individuals across all areas of the county, aiming for as many as
possible to return to their own homes.
 
Following a compliant procurement process, the
Council, acting as the commissioning lead on behalf of the health
and social care system is now recommending award of the following
Hospital Discharge Contracts:
 

An additional 4 block Hospital
Discharge Care (HDC) contracts, providing an increase of 575 block
hours per week for a collective total estimated cost of £265k
from December 2023 until March 2024 with potential to extend until
March 2025 at an estimated cost of £810k (subject to
additional funding and demand).
A block hospital discharge contract
in Worthing at an estimated cost of £280k from April 2024
until March 2025, to be awarded subject to funding commitment.
9 domiciliary care rounds at an
estimated cost of £195k from December 2023 until March 2024
with the ability to extend up to March 2025 at an estimated cost of
£590k (subject to additional funding and demand).

 

Decision

The Director of Adults and Health has approved
the award of the Hospital Discharge Contracts to organisations,
listed in the decision report at table 2.4, for an initial period
of four (4) months with the option to extend for 12-months until
March 2025 if performance and delivery of the contracts are
satisfactory, and funding commitment is provided from the
Integrated Care Board.
