Report in relation to the Council’s contribution and
support for the review of hospital discharge and community services
in partnership with health

Decision

To approve the Council’s contribution and support to a
programme of transformation to be undertaken in partnership with
NHS Wrightington, Wigan and Leigh Teaching Hospitals Foundation
Trust and NHS Greater Manchester Integrated Care relating hospital
discharge and community services pathways.
