Purpose

To provide
funding in partnership with the Greater Manchester Integrated Care
Wigan Locality to support Dementia Advisors and Carers Support
Service contract. This will support wellbeing for people living
with dementia and their carers to live well. Focusing on the
principles of resilience and independence, strengthening community
connections and transforming models of support to divert people
away from inappropriate dependence on formal health and social
care.
The future contract for this service will be held as an NHS
contract by Greater Manchester Integrated Care Wigan
Locality
 

Decision

 The Director -
Social Care and Transformation authorises:
 

(1)    
the new approach to the
commissioning of the Dementia Advisors and Carers Support Service
including the contribution of funding to the GMICB in this
respect;

(2)    
the Assistant Director - Legal,
Governance & Elections to enter into such documentation on
behalf of the Council, that may be necessary to give effect to 1
above;

(3)    
agreement that the contract moving
forward will be held as a NHS contract by Greater Manchester
Integrated Care Wigan Locality as they provide the greater share of
the funding, and that Wigan Council will continue; and

(4)    
support of the funding of the
Dementia Advisors and Carers support service for the next three
years.
