The Lambeth Together Care Partnership Board scheduled a meeting to discuss updates on key developments in Lambeth's health and care services, including the Living Well Network Alliance, the Lambeth Together Assurance process, and neighbourhood working. Neighbourhood Working
The board was scheduled to discuss an update on neighbourhood working in Lambeth, including the Lambeth Integrator Model, the Integrated Neighbourhood Teams (INT) Delivery Plan, and the South East London (SEL) Ageing Well Framework.
The Lambeth Integrator Model is a partnership between Guy's and St Thomas' NHS Foundation Trust (GSTT) and the Lambeth General Practice Provider Alliance (LGPPA) that aims to lead the delivery of integrated neighbourhood teams. The board was asked to ratify the proposed model, approve the use of £250,000 for integrator development, and delegate final approval for the integrator memorandum of understanding (MOU) to the Neighbourhood & Wellbeing Delivery Alliance Leadership Board.
The Lambeth INT Delivery Plan for July to December 2025 outlines the development of integrated neighbourhood teams across Lambeth's five neighbourhoods:
- North Lambeth & Stockwell
- Clapham
- Brixton and Herne Hill
- Streatham
- Norwood
These teams will focus on specific population cohorts, including frailty, multiple long-term conditions, children & young people, and mental health. The board was asked to endorse the INT Delivery Plan.
The SEL Ageing Well Framework, developed as part of the ongoing development of integrated neighbourhood teams, aims to create age-friendly communities and improve health outcomes for older adults. The board was asked to ratify the framework.
Lambeth Together Assurance Process Review
The board was scheduled to discuss the Lambeth Together Assurance Process Review for 2025/26. The review aimed to recalibrate the impact measures used to track delivery against the outcomes set out in the Our Health, Our Lambeth plan1, review and recommend improvements to the process for gathering the assurance narrative, and strengthen alignment across various Lambeth Together groups. The board was asked to approve proposed changes to the Health and Care Plan impact measures for 2025/26, approve the forward view presentation timetable for 2025/26, and provide feedback on any additional steps that could be taken to improve or streamline the assurance process.
Living Well Network Alliance (LWNA) Roadmap
The board was scheduled to discuss the Living Well Network Alliance (LWNA) Roadmap. The LWNA aims to improve the mental health and wellbeing of Lambeth residents by integrating clinical, social, and voluntary sector staff into neighbourhood-based Living Well Centres. The roadmap includes a new focused set of outcomes and priorities, including reducing crisis presentations for people known to services, increasing early support for Black people, and improving people's recovery outcomes. The board was asked to endorse the Alliance's agreed focus on outcomes and support the implementation of the roadmap for the next 12 months.
Healthwatch Lambeth Annual Update
The board was scheduled to receive an update on Healthwatch Lambeth's impact, community engagement, and contribution to service improvements across 2024 - 2025. Healthwatch Lambeth is the local health and social care champion, ensuring that people's voices are heard and used to improve services.
According to the report pack, Healthwatch Lambeth supported over 23,400 people with care information and advocacy, shared 830 experiences to improve local health and care services, and published 39 reports, including on pharmacy access, digital inclusion, and mental health.
Care Quality Commission (CQC) Assessment of Lambeth Adult Social Care (ASC)
The board was scheduled to discuss the Care Quality Commission (CQC) assessment of Lambeth Adult Social Care (ASC). The CQC is undertaking initial assessments of local authority Adult Social Care (ASC) services nationally, which will yield a public judgement using the same one-word
judgement outcomes as Ofsted.
The board was asked to note that the CQC has commenced its assessment process of Lambeth Adult Social Care in May 2025, read Lambeth Adult Social Care's Self Assessment Executive Summary, and engage with CQC assessors as part of the assessment fieldwork.
Primary Care Commissioning Committee (PCCC) Update
The board was scheduled to receive an update on discussions held at the Primary Care Commissioning Committee (PCCC) on 14 May 2025, and ratify decisions made at that meeting. The PCCC is responsible for facilitating accessible and equitable primary care services in the community, including services provided by General Practices, Dentists, Pharmacists, and Eye Care Specialists.
The PCCC approved decisions relating to Lambeth Estates, the Lambeth Offer & Commissioning Intentions, SEL ICB Primary Care Policies and Guidance, GP Out of Hours for a Lambeth Practice, and APMS legal advice. These included:
- Pursuing single site solutions for the Lambeth Walk Group Practice and Waterloo Health Centre and supporting the potential to collocate the Lambeth Walk Group Practice with another surgery.
- Consolidating the Edith Cavell and Streatham Place at the Streatham Hill Primary Care Centre.
- Providing additional rent reimbursement to support the extension of the Stockwell Group Practice.
- Approving the SEL Primary Care Managing Late/Retrospective Claims Policy, the Local Dispute Resolution Process, and the short-term financial support guidance documents.
- Issuing a second breach for non-compliance with the initial remedial notice to a practice in Lambeth regarding GP Out of Hours service provision.
- Approving the recommendation to proceed with award at risk in response to a challenge to a procurement process.
Place Executive Lead Update
Andrew Eyres, Place Executive Lead Lambeth, Corporate Director, Integrated Health, and Care, Lambeth Council and South East London Integrated Care Board, was scheduled to provide an update on key developments since the Lambeth Together Care Partnership Board meeting in public on 15 May 2025. The update included information on the Our Health, Our Lambeth plan, the Board on the Bus programme, Clinical and Care Professional Leads, NHS Transformation, System Pressures, and Delivery Alliances.
According to the report pack, the Lambeth Together Executive Group received updates on the activities of Clinical and Care Professional Leads (CCPLs) from the Neighbourhood and Wellbeing Delivery Alliance, including work on early Cancer Diagnosis, supporting GP practices in utilising the chronic pain register, piloting holistic Frailty support, and Engaging with Communities.
The report also noted that Integrated Care Boards (ICBs) across England were asked to reduce their running costs by, on average, 50% by the end of the 2025/26 financial year, and that South East London Integrated Care Board (SEL ICB) submitted its high-level proposal for implementing the target operating model.
Lambeth Together Assurance Sub-Group
Jasmina Lijesevic, Lambeth Together Board Lay Member, and Warren Beresford, Associate Director Health and Care Planning and Intelligence, were scheduled to provide a report from the Lambeth Together Assurance Sub-Group and the associated Integrated Assurance Report presented on 13 May 2025. The Lambeth Together Assurance Group (LTAG) meeting agenda centred around the outcome: People have access to joined up and holistic health and care delivered in their neighbourhoods.
