extension of the Short-Term Homecare contract. 

Reasons for the decision

The utilisation of the permissible one-year
extension on this Short-Term Homecare contract Is approved due to
the previous 4 years outcomes being evidentially beneficial in
support of the whole Health and Social care system (discharge and
hospital avoidance). Furthermore, this complements the
Council’s own reablement team with additional
capacity.
 
By deploying the further year extension,
different models in support of Discharge to Assess will be explored
in readiness for commissioning activity for July 2027 when the
contract fully expires.
