Hammersmith and Fulham Health and Care Partnership Update
The committee was scheduled to receive an update on the progress of the Hammersmith and Fulham Health and Care Partnership. This partnership aims to improve health and wellbeing across the borough through collaborative working and integrated care efforts. A key focus of this work has been the development of the Integrated Community Access Point (ICAP), designed to transform care for individuals with complex needs by improving access and personalising support. The partnership's approach is guided by the 10 Year Health Plan and focuses on integrating services, creating health within communities, and working with local people to co-produce service changes. The report was expected to detail progress made since the partnership's refresh in September 2024, acknowledging challenges such as financial pressures and a fragmented NHS provider landscape, while highlighting ambitions for future integration and collaboration.
H&F Safeguarding Adults Board Report 2024/25
The committee was scheduled to review the annual report of the Hammersmith & Fulham Safeguarding Adults Board (H&F SAB) for the 2024/25 period. This report was intended to outline how the board has fulfilled its statutory duties, including summaries of Safeguarding Adults Reviews (SARs) conducted under Section 44 of the Care Act. The report was also expected to detail the progress made against the board's three key priority areas: effective systems and processes, creating a culture of learning, and communication and partnership. The report highlighted proactive efforts by partners to improve responses to adults at risk of abuse and neglect, and indicated that the SAB was moving into the final year of its three-year strategy, with a focus on reviewing impact and strengthening assurance mechanisms. The report included details of newly commissioned SARs, such as those concerning Adult D,
a thematic review of suicide, Adult E and Adult F,
a thematic review of chemsex, and Adult I.
Charing Cross Hospital Co-Production
An update was scheduled regarding co-production work at Charing Cross Hospital1, following a review by Action on Disability titled Disabled People's Experiences of the NHS.
This review, which gathered views from 26 disabled people, highlighted issues such as poor signage, long waiting times, physical barriers, and limited wheelchair access. The report was expected to detail the hospital's response to these findings and the progress made in collaboration with Action on Disability. This included site visits in March 2025, co-production meetings in May 2025, and engagement with architects during the concept and detailed design stages for redevelopment. Key areas of focus included improving physical accessibility, wayfinding, volunteer support, sensory environments, and staff awareness training.
Drug Strategy
The committee was scheduled to comment on the upcoming Hammersmith and Fulham Drug Strategy for 2025-2028. This strategy builds upon the work of the Hammersmith and Fulham Combatting Drug Partnership, established in 2022 following the Dame Carol Black Review and the national 'From Harm to Hope' strategy. The proposed strategy is structured around three key themes: breaking drug supply chains through enforcement, providing world-class treatment services by treating addiction as a health problem, and achieving a generational shift in the demand for drugs by working with young people to change attitudes towards drug-taking. The report highlighted that Hammersmith and Fulham has one of the highest drug-related death rates in London and that the strategy aims to reduce drug supply, tackle drug-related crime, and reduce substance misuse-related harms.

The committee was also scheduled to approve the minutes of its previous meeting as an accurate record and to note any outstanding actions.
